Case Study · City of Littleton
KTI partnered with the City of Littleton and a senior residential community to create a pathway to permanent housing and ongoing health supports for residents living in housing instability. The LME system gave residents a safe, continuous way to share what they were experiencing and that data went directly to social workers and city leadership. The data exposed current needs and established a health baseline. Over the next year KTI continued following up with residents, LME amplifying voices and sending boots-on-the-ground alerts to social workers so they could get residents what they needed, when they needed it.

When a resident says “I’m worried about paying rent” or “I skipped my medication this week,” the platform auto-triggers the appropriate SDOH Medicaid billing codes — for any Medicaid service, not just housing.
